I understand that there are inherent risks involved in any ministry, event, outing or trip, and hereby release Cornerstone Church, it's pastors, employees, and volunteers from any and all liability for any injury, loss, or damage to person or property that may occur during the course of my or my child's involvement.
In the event that there is an illness or injury that requires the attendtion of a doctor, I consent to any reasonable medical treatment as deemed necessary by a licensed physician including anesthesia and surgery. In the event treatment is required from a physician and/or hospital designated by the Church, I agree to hold such person free and harmless of any claims, demands, or suits for damages arising from the giving of such consent. I also acknowledge that I will be ultimately responsible for the cost of that medical care should the cost of that medical care not be covered by my health insurance provider.
Further, I also agree to bring myself or my child home at my expense should an illness or injury require it.
I also give permission to be photographed and/or recorded for promotional purposes of Cornerstone Church.
